If you want to focus on a specific goal like housing, you can use your Support Coordination funding to work with a coordinator who understands housing supports. They’ll guide you in finding the right services and help you take steps toward your goal.

Who is eligible for Support Coordination under the NDIS?

Support Coordination is funded for participants who need extra help navigating their plan. Not everyone automatically receives it, but you can request it during your plan review if you think it will benefit you.

What if I need help with daily activities in addition to nursing services?

We offer personal care services in addition to community nursing, which can help with daily activities like bathing, dressing, eating, and mobility. Our team will coordinate with you to provide a comprehensive support plan that addresses both your health and personal care needs.

What’s the difference between Support Coordination and plan management?

Support Coordination helps you connect with and manage your supports. Plan management handles the financial side, like paying providers and keeping track of your NDIS budget.
